Community Health Workers (CHW)

Four Corners Health Department in York, NE is hiring Community Health Workers (CHW) to help support people in Butler, Polk, Seward and York Counties. In this role, you will work closely with individuals and families, assist them find helpful services, and share information that supports healthy living. JOB SUMMARY:
Community Health Workers are responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. CHW activity will promote, maintain, and improve the health of clients and their family. CHW will provide social support, advocate for individuals and community health needs, and provide services such as blood pressure screening. Community outreach, such as teaching community classes, presentations, home visits and health screenings, may be required. Community Health Workers operate in a non-clinical, relationship-centered role and do not provide medical diagnosis or clinical decision-making. This is a non-exempt position.
