JOB SUMMARY:
Community Health Workers are responsible for helping clients and their families to navigate and access community services, other resources, and adopt healthy behaviors. CHW activity will promote, maintain, and improve the health of clients and their family. CHW will provide social support, advocate for individuals and community health needs, and provide services such as blood pressure screening. Community outreach, such as teaching community classes, presentations, home visits and health screenings, may be required. Community Health Workers operate in a non-clinical, relationship-centered role and do not provide medical diagnosis or clinical decision-making. This is a non-exempt position.
